Two paragraphs:

Note: Do not make this agreement with an alcoholic unless he/she is already attending weekly meetings. An alcoholic is incapable of keeping such an agreement with you.

1. "I am clear that I am sick and addicted to abuse. You have my word that I will attend weekly therapy, support group, or counseling sessions, until you are satisfied that I have healed myself."
   

2. "If I break my word and stop going to therapy, for any reason, if I miss even one session, for any reason other than an emergency, you have my word I will stay away from you physically, verbally, and in writing, until I have completed six months in a row of weekly therapy sessions."
